    Abstract:   本研究探討安寧病房癌末病患以身體為基礎對病情進展與告知的知覺經驗。以詮釋現象學為研究方法,深度訪談七位臨終病人後,以訪談逐字稿及田野觀察記錄為文本,透過視域化、形成主題經驗範疇及構面的過程,進行資料的詮釋分析。然後依此呈現臨終病情身體知覺經驗的深度描寫與意義詮釋。   研究結果顯示,臨終病情可從「身體知覺經驗」及「臨終知覺經驗意義」兩個主軸來進行理解。第一個主軸分從四個層次加以說明:一、身體病覺經驗;二、身體知覺世界的裂解;三、身體時間感的失序;四、存有世界關係的解離。第二個主軸分從四個層次加以說明:一、病情告知的體察;二、死亡焦慮的覺察;三、存在孤獨的體會;四、瀕死覺知的靈性現象。   本研究發現身體知覺經驗是臨終病人對病情體驗的基礎,而由病人知覺經驗開展的病情理解是病情告知者必備參考。臨床上可縮短醫病之間認知上的落差,以及提供回應存在關係裂解、死亡焦慮、陪伴關係、瀕死覺知等臨終照顧議題的洞見。
      This study, on the basis of body experience, explored how terminally ill patients in hospice perceive the progression of their own ill condition as well as “truth-telling.” Guided by hermeneutic-phenomenological research method, this study conducted in-depth interviews on 7 terminal patients. The recorded interviews were then transcribed into verbatim text. Along with the field notes, the text was analyzed and interpreted by way of forming horizon and thematized categories and dimensions of experience. The terminal patients’ perceptual experience towards their own ill condition was, therefore, delineated and interpreted.   The research results demonstrated that terminal ill condition can be understood from the following two axial themes: 1.bodily perceptual experience, 2. the meaning of terminal perceptual experience. The first theme can be illustrated by the four dimensions: 1.bodily awareness of illness, 2.breakdown of bodily perceptual world, 3.disorder of temporal perception, 4. disassociation of worldly relations. The second theme can be illustrated by the four dimensions: 1.awareness of truth-telling, 2.awareness of death anxiety, 3.awareness of existential solitude, 4.spiritual phenomenon of dying awareness.   This study found that bodily perceptual experience is the basis of terminal patients’ awareness of their own ill condition and that it is necessary for medical staff to refer to patients’ understanding of their own ill condition as they conduct truth-telling. Clinically, this study can shorten the gap between medical staff’s and patients’ recognition of illness. It can also provide insights into a variety of issues regarding terminal care such as “breakdown of existential relationship,” ”death anxiety,” “accompanying relationship,” and “ dying perception.”
