本研究以財團法人國範文教基金會為對象,透過個案研究法,藉由與專家學者與實務工作者的深度訪談,回顧該非營利組織透過政策倡導推展全人健康照護理念的歷程,並就執行成效進行評估,另就該非營利組織透過社會倡導提升民眾健康知能之案例進行分析,探討衛生醫療領域的非營利組織在健康照護議題上所能扮演的角色,並提出衛生醫療領域的非營利組織未來在全人健康照護議題開拓發展空間的建議。 透過文獻及質性訪談,本研究發現:由於臺灣人口老化,慢性病盛行,失能人數不斷增加,民眾對於全人健康照護的需求極為殷切,全人健康照護系統的運作強調扎根社區且以人為中心,在此一議題上,衛生醫療領域的非營利組織能夠經由政策倡導與社會倡導的積極參與,與相關政府單位及教育機構進行各方面的連結,在全人健康照護系統的制度建置、人才培育及民眾健康知能提升等方面,扮演重要的角色。 依據上述發現,本研究就衛生醫療領域的非營利組織未來在全人健康照護議題開拓發展空間,提出下列建議:(一)在制度建置方面,非營利組織可以透過政策倡導,推動以「社區醫療群」為基礎的家庭責任醫師及健保論人計酬制度,為全人健康照護系統奠定穩定基礎。(二)在人才培育方面,非營利組織可以透過與教育機構合作,強化以「一般醫學訓練」為主軸的全人照護醫師培育,實踐醫療專業盡責,提升醫療照顧的品質與安全。(三)在民眾健康知能提升方面,非營利組織可透過社會倡導機制,強化民眾就醫知識及用藥安全,加強個人自我健康照護的能力與責任,減少民眾的健康差距。此外,為因應高齡化社會的健康照護需求,非營利組織可以與地方政府、醫療機構、學術團體或公民團體合作,共同推展運動保健模式,協助民眾透過終身學習、社會參與或志願服務,來實踐「健康老化」的理念。 Non-profit organizations can play an important role in shaping the health care policy. Our society is in increasing needs of holistic health care as the demographics of Taiwan are quickly aging, People/population suffering from chronic diseases are increasing, and the disabled people is growing. In response to the health needs of these populations, non-profit organization is hoped to actively participate in advocating for the health care policy as well as in improving health literacy. This study takes Guofan Foundation as concrete example to highlight the important role non-profit organizations can play in advocating for the health care policy and in improving health literacy. Results provide evidence that policy-related activities can be undertaken by non-profit organization in the field of holistic health care. Non-profit organizations can cooperate directly with government agencies or educational institutions in this field to achieve their missions. In light of these results, some strategic approaches are advised to expand the advocacy role played by nonprofit organizations in the health care sector. It is our suggestion that nonprofit advocacy for holistic health care may include establishment of an accountable family physician system to improve community health, sustaintment of educational reform in holistic physician training to improve health care quality and professional accountability, enhancement of health literacy to promote the quality of life in a community, as well as participation in activities contributing to healthy aging.
