    Abstract:   本文透過Google文件表單的設計與填寫,來探究線上調查結果如何影響及改變國小中年級學生的工作態度。我們先觀察選定合適的個案進行網路問卷調查,再透過訪談、進一步觀察與調查並應用行為改變技術促成個案的工作態度改變。透過讓國小中年級學生進行Google文件表單設計之自評與互評及統計圖表,我們可探索網路調查工具對工作態度改變的影響。   從研究結果發現個案學生能在填寫好線上自評與互評表單及其他表單,並看過結果之後,於生活中對其偏差的工作態度有所改變。最後,本文的研究結果可以提供給學生家長與國小教師作為教導孩子、糾正並培養正確工作態度的參考。
      This thesis uses the design of Google Docs Forms to investigate how the results of online surveys will influence and change the attitude of middle-grade elementary school students toward work. Students who are suitable for the case study are surveyed on the Web along with interviews and further observation. We also use the behavior modification techniques to help modify their attitude toward work. The statistical charts of self-assessments and peer-evaluations might show how the online surveys help modify students’ attitude toward work.    The research results of this thesis show that the students in the case study are able to fill out the self-assessments and peer-evaluations, or other online forms. After reading the result charts, the students could have attitude changes toward work in the real classroom. Finally, the research results could help elementary school students’ parents and teachers to get some suggestions in teaching or developing students’ appropriate attitude toward work.
