    Abstract:   本研究的主旨是在探討癌末病患與家屬之情緒轉換歷程,以瞭解癌末病患與家屬的情緒本質與內涵、過程中的困難與轉化機會及相關的影響因素。本研究的場域在南區某區域教學醫院的安寧病房,本研究的方法是採立意取樣,病患和家屬各一人(共3組),以參與觀察和訪談法進行資料收集,並以質性研究的解釋性互動論作為研究結果的分析方法。    研究結果顯示:(1)癌末病患與家屬情緒轉換歷程呈現「震驚-無奈」、「安頓-變動」、「混亂-轉化」三個階段,病人以直接性體驗自己的身體變化,家屬經由關注而感受病人的身體變化;轉折點能使病人與家屬強烈的感受到死亡,震驚的情緒能產生急迫感,使得病人與家屬開始面對病人的死亡;轉換的過程是在不斷地改變中進行,使病患與家屬逐漸能在情緒上接受病人的死亡。(2)病患與家屬隨著病情變化,在身體、時間、空間和關係四個向度裡轉變。病人在身體的變化中感受癌症所帶來的痛苦與死亡威脅,內在時間感與生活空間跟著產生變動,強烈的負向情緒,使被愛、被瞭解和被接納的需求增加,與他人產生連結性關係亦變得重要且迫切;家屬則經由看見而覺知病人的痛苦與身體變化,家屬也因此而感覺痛苦,家屬對病人的情感,使家屬的生活依隨病人而變動,情感的痛苦也使得家屬轉向病人尋求情感的連結。(3)在趨向轉化的過程,病人經由病痛形成對自己的持續關注,使病人面對自己,並在面對自己的過程內在發生轉化;家屬經由看見的過程感受死亡,強烈的無常感使家屬更認真的看待自己的人生,情感的受苦使家屬看見自己對病人的愛以及自己對愛的渴望;病人與家屬在此過程裡的負面情緒是人在抗拒改變與趨向改變之間的自然產物。(4)病人與家屬在面對自己的過程,於抗拒與接受改變之間顯現個人內在深層的痛苦與渴望,同時也促使人朝向內在進行轉化,接受無可抗拒的命運和痛苦、接受生命的本然,對自己與他人升起悲憫、理解與接納。   根據以上結論,本研究提出相關建議,提供臨床工作者與未來相關研究的研究者參考。
      This study aimed at the exploration of the emotional transition process of terminal cancer patients and their families, through understanding the nature and meaning of the emotions, to discover the difficulties and opportunities of related factors. This study used purposive sampling method to recruit three cases each includes one patient and one family member, the researcher(an experienced former nurse) stay with them for more than one mouth, to conduct participating observation and interview in the setting of the hospices ward. Interpretive interaction method was used for analysis.    Four conclusions were found:(1)The emotional transition process of terminal cancer patients and their families showed a three- stage response; which is〝shock but not able to avoid〞,〝stability of change〞and〝confusion to being〞. The patients experienced immediate body change towards death, the families experienced a spectator role to see the patients coming to an end;turning points in the patients and their families could caused a strong feeling towards death, forcing patients and their families began to face the death. The transition process was a stage for change which prepared patients and their families to accept the death of the patient in the emotional level;(2)The patients and their families shifted along four aspects, which included the issues of body, time, space, and relationship. The patients by suffering the death form the body, increased their need of being loved, being understood, and being accepted; whereas the families by witting patient’s suffering, caused them the shift of emotional link from confronting to compromise;(3)The patients through the illness continued to form his-own new concern , so to inform their internal transformation. The families, through “looking” the approaching of the patient’s death also uncovering their innate love towards the patients.(4)Patients and their families moved slowly in this changing process of resistance to achieve their own inner transformation, to accept the fate of life by understanding and acceptance. Suggestions were made according to the results.
