    Title: 加護病房護理人員工作心路歷程之探討

    Abstract:   以「提供病人照顧」為主軸的護理相關訓練,通常只期待專業照顧者的角色具有全面性及完整性照顧,而忽略了護理人員個人的需求,特別是情緒上的需求。服務於醫療院所高死亡率部門(如癌症病房、安寧病房、加護病房等)的護理人員,長時間面對瀕死及死亡會產生憤怒、焦慮不安、失望、無助感、無力感等情緒反應,因此,研究者想進一步探問,在以強調理性和控制為導向的醫療體系中,護理人員情緒的出口在那裡?護理人員如何在面臨病患死亡經驗中找出因應之道?協助護理人員觀看自己的故事,探討經由照顧經驗過程所形成的一些主觀意識,期望透過加護病房護理人員的經驗描述呈現出加護病房護理人員的心路歷程與悲傷反應及調適。    本研究將採用質性研究方法,利用深度訪談及半結構式問卷來蒐集資料,以敘事分析(narrative analysis)為研究取向,以「整體—內容」的分析模式呈現三位研究參與者悲傷故事之探討;以「類別—內容」的分析模式針對研究參與者在各自的工作經驗、情緒感受及思考生命意義價值的取得進行歸類與整理。    本研究提供下述幾點建議作為參考:一、為提升遺體護理的部份提升實作的能力,在學校教育中應有對遺體護理的技術操作及注意事項,並且列為技術考的項目。二、加強諮商輔導的在職教育課程及生命教育的課程宣導。三、增加家屬的溝通管道,讓家屬可以在對病情有所疑問或情緒上需要所支持時可以有一個管道可以尋求四、建議成立醫院的心理支持團體或輔導諮商中心。 五、加強社會大眾生命教育及臨終關懷的宣導。
      The nursing correlation training takes "providing cares for patients" as the main axle, and it usually just anticipated the specialized attendance role has comprehensive and the complete attendance, but neglected the individual demand in nursing staff, especially in mood demand. The nursing staff serves to departments in a hospital with high mortality rate (for example: oncology ward, hospice ward, intensive care unit, and so on), faces on pre-dying and death for a long period, would present as anger, anxiety, disappointment, hopelessness, incapable feelings, and so on. Therefore, researcher wants to inquire farther, in the medical system which takes emphasizes the rationality and the control as guidance, where is the mood exit for nursing staff? How does the nursing staff find the coping strategies while facing to the experience of the patients’ death? We helped the nursing staff to observe their own story, discussed some subjective consciousness which forms by way of the attendance experience process, and then through the nursing staff's experience in intensive care unit, we expected to describe the psychological experience, grief response and adjustment of them.    The study will assume the method of qualitative research and collect the data by depth interview and semi-structural formula, and then take narrative analysis as the research orientation, to present the sad stories of the three research attendants by the analysis pattern "whole — content"; to classify and recognize the view of attendants at respective working experience, the mood feeling, and the thought on value of life significance by the analysis pattern "category — content."     This research provides the suggestions described below for reference:1.In order to promote the ability of actual operation in body nursing, the technical operation and the matters need to be noticed about body nursing should be taught in the school, and lists as the project in the technical tests.2.Strengthens the counseling guidance in office education courses and the life education curriculum 3.Increases communication channels for family members, make them to have a channel to quest for question in disease condition or the support of mood. 4.To establish psychological support association or counseling center in hospital is suggested.5.To strengthen social populace’s guidance of life education and the deathbed care.
