| 摘要: | 本研究以九十二年，雲嘉南地區辦理「非中低收入失能老人補助使用居家服務試辦計畫」為研究對象，共獲得186位老人、109主要照顧者、107位照顧服務員，以及10位機構主管訪談所得與業務月報表等相關資料。透過品質、效率及效益三方面來進行方案評估，了解目前該方案的推行與執行結果。而且研究透過各非營利組織在方案投入項與產出項進行資料包絡分析，求得生產效率、技術效率，以及規模效率；運用該方法的差額變項分析尋求無效率之非營利組織最適生產力參考，以便作為部門經營、管理改善績效之具體參考依據。研究結果顯示： 效能方面：一、照顧服務偏向提供個人照顧(Personal Care)，但是對於老人社會參與或知識資訊的傳遞上仍然不足。 二、照顧服務工作協助主要照顧者減輕「照顧者工作分擔」與「家庭喘息」兩方面。但是在發展社交生活的機會仍然不足，這可能與照顧關係與補助時數有關。 品質方面：一、大多數的老人對於照顧服務員所提供的服務項目感到滿意。 二、老人對於照顧服務員的服務品質滿意程度仍然很高，但是因為縣市別的不同，會產生些許差異。 三、嘉義縣與雲林縣在服務整體的工作滿意度表現最好，台南縣要在「工作認同」、「在職教育」、「機構體制」，以及「工作福利」仍有待改善。 效率方面：一、應用資料包絡分析得知：非營利組織無效率的主因來自於二方面，第一是個案量的不足，其次是「完全自費」與「部分自費」個案量太少導致。 二、建議非營利組織個案量不足時，宜採增加個案量以提高效率；但是在個案以達一定數量之後，開發「完全自費」與「部分自費」個案，減少對於政府資金補助的依賴。 依據研究結果提出應用與建議如下：一、持續推展與宣導照顧服務與使用者付費的觀念。 二、配合社區工作推展與民眾建立信任有好關係。 三、建罝照顧服務員工作制度與權益。 四、建立完善的督導與評估制度，確保服務效能與品質。
This research is about Yun-Lin, Chia-Yi and Tai-Nan areas promoted Fee-for-Service home care programs in 2003. In the research process, got 186 samples of disabled aging people, 109 caregivers who take care mainly, 107 health care aides, and 10 administrators accepted interview and got monthly report during 2002. In this research, I tried adopted three dimensions to evaluate program including effectiveness, quality and efficiency. Find out about the implementation of this program and execution result at present.In Order to know the efficiency performance I applied DEA (data envelopment analysis) examine input and output data from each of nonprofit organization that promoted fee-for-service of home care programs in Yun-Lin, Chia-Yi and Tai-Nan areas, then it’s also be called DMU (decision making unit). By DEA analysis, I get production efficiency, technological efficiency, and scale efficiency. The results also suggested efficient reference set for those relative inefficient studied subjects. Hopefully the following finding will provide some basic reference about administrative manage and improve program performance. Effectiveness:1. Care- service is only to offering personal care, but about the social participation and the knowledge information is had still insufficient. 2. Care- service succeed in helping mainly caregiver reduce burden from .sharing responsibility for care disabled aging people and respite care of family. But the chance to develop social life is still insufficient, this may relate to who take care and subsidize the hours. Quality: 1. most old men feel satisfied with home care service items. 2. the old man consider that home care aides service quality satisfaction is above satisfaction. But because it is some differences among three counties. 3. Home care aides working satisfaction that Chia-yi county and Yun-Lin County got best performance, but Tai-nan have to improve in job identification ,on-the-job training, organization's institutionalization, and the work welfare. Efficiency:1. use DEA to learn : the main reason inefficient in nonprofit organization comes from two respects, the first is deficiencies of the case quantity, secondly it is result from user fee amounts too few. 2. Second, propose nonprofit organization should adopt and increase the case amount in order to raise the efficiency when being insufficient in case amount; But in order to be up to some in the case , develop and advocate user fee reducing the reliance that is subsidized to government's fund. According to the result of study, propose as following: 1. Let’s continuously promote and declare care service industry. 2. Nonprofit organizations cooperate with community's work and promote and establish trust relationship with the people in the local community. 3. Government should build home care aides institutionalization, and protect home care aides work environment rights and interests. 4. Government set up perfect supervising and appraisal evaluations, guarantee to serve the effectiveness and quality. |
