台灣地區衛生體系出生通報資料全國性之建置,自2002年05月起提供接生單位經由上網通報後,其通報比率至2004年4月已接近100%,對於通報資料之建置匯集亦急速加快與完整,而且資料登錄品質經由程式提供線上檢核與輸入者互動,也比以往紙本通報大幅提昇。在衛生署主管機關國民健康局管理端,除了提供通報網站系統建置平台與維護外,亦定期下載資料至後端衛生體系出生通報資料管理資訊系統,並進一步作資料完整性之相關性檢核確認,此後端出生通報資料管理資訊系統(management information system)主要是提供衛生公務統計表報與出生資料庫長期性及完整性之管理,並提供產業界與學術界申請做為研究相關議題之資料來源。 本研究以建構台灣地區衛生體系出生通報資料之資料倉儲系統(data warehousing system),經由其後端平台衛生體系出生通報資料管理資訊系統,擷取相關欄位經轉換及整合等程序,建構出生通報資料倉儲,並再進一步發展成資料倉儲系統。在系統實作上規劃數個現階段需求之國民健康資訊主題,讓資料倉儲系統使用者藉由操作簡便介面,即可獲得衛政決策輔助性參考資訊,並預留後續導入實務擴充發展之彈性,是故,將可增加衛生體系出生通報資料之運用與價值性。 因此,本研究的研究目的有二項:壹、經由建構資料倉儲系統之開發模式,以台灣地區衛生體系出生通報資料建置為例,試擬提供未來其他國民健康資料庫發展資料倉儲系統之參考模式。貳、經由提出一些國民健康資訊主題之需求,規劃出生通報資料倉儲雛型系統之功能,讓使用者藉由操作簡便介面,即可獲得國民健康資訊。因此,本研究提出之國民健康資訊主題有三類:(一)提供有關摘要性之國民健康資訊;(二)提供與妊娠結果相關之特定國民健康資訊;(三)提供有關產婦此次生產前後在醫學上初步所見特殊情形之國民健康資訊。 本研究之結論有三項:壹、提供給建置相關資料倉儲與資料倉儲系統模式時參考。貳、對於出生通報資料提供簡便操作程序,即能獲得國民健康資訊。參、增加出生通報資料運用上之效益。本研究對於未來發展之建議有四項:壹、未來擴充導入其彙總性之子資料倉儲系統;貳、未來可整合上下游資料發展成具有決策性支援之資料倉儲系統;參、考量當資料來源在異質資訊平台時相關的前置處理之因應作業;肆、進一步改良系統部份功能執行上費時之程式提高其處理效率。 The birth report rate improved dramatically since May 2002, which required the deliverers of the babies to register the birth through web. After establishing the on-line birth registration system, the demands for faster integration of related information and ensuring higher quality of data are increasing. The Bureau of Health Promotion who in charge of the on-line birth registration system not only have to verify the completeness and accuracy of birth registration data, but also have to setup a management information system to provide relevant information for public health tabulation, long-term monitoring the neonatal status for health policy planning and academic researches. In this study we established a data warehousing system, which composed different data-warehouses each of them were relevant to specific health theme. In the end plate of birth registration management information system, we caught related variables through the transformation and integrating procedures and ensured that the interface should be user friendly. We also left enough space and flexibility for future enlargement and adding further needs to provide a decision making assistant system. There were two aims in this study, the first was to establish a data warehousing system using the birth registration as an example and shared the experience for other related topics. The second was to propose different kinds of health related demands and developed an archetype for reference. Through a user friendly interface the relevant information could be get as needed. Three health-related themes were proposed: 1) summery health indicators of population health; 2) birth outcome related information; 3) maternal related information before and after delivery. In conclusion, it was feasible to establish a data warehousing system to provide relevant population health related information. Through our management information system, the accuracy and efficiency of getting birth registration information were improved. The convenience to the users also improved through our system. Suggestions for future studies included: 1) enlarging the data warehousing system to integrate relevant information to increase the function; 2) combining the upstream and lower stream data need to establish a decision making support system; 3) developing a operating procedure when encountering the different information formats; 4) improving the systematic function to increase the management efficiency.
