Abstract:	基層衛生所功能重整、組織轉型一直是公共衛生界重大課題，如何強化衛生所的功能、組織如何轉型，當今需慎思。過去基層衛生所因有政府足夠的預算支持，尚不覺生存競爭之威脅，如今在政府預算逐年減少之同時，自應提出具體數據與分析，作為未來政府著手改善基層衛生所轉型之重要參考依據。　　　基層衛生所對公共衛生之推展與配合上及醫療政策推動有其長久之貢獻；但因受到組織老化、人事、會計制度僵化等因素之束縛，而無法應變健保總額制度下的醫療競爭壓力與公務預算短縮之狀況。在諸多轉型方案中以行政法人之模式較為適宜，希望衛生所行政法人後藉由遴聘社會專家學者成立董事會；新進人員不具公務員身分；會計、採購上，監督機關由事前監督改為事後審查以及在公共衛生之推展與醫療政策之配合保有國家衛生政策推行；對基層衛生所競爭力的提升有實質的助益並能減輕政府的財務負擔。　　The primary health stations function restructure, organizational transformation to have been the public hygiene boundary major topic, how enhance the function of the primary health stations, organization how transform, need to be thought deeply nowadays. Pass by primary health stations because of having the government enough budget support, don't feel the threat of the existence competition still, reduce year by year in the government budget now it put forward the concrete data and analysis it will of course in the meantime, be a government to will begin an important reference of improving the primary health stations transformation in the future according to.　　　The primary health stations promotes to ascend with concert to the public hygiene and the medical treatment policy promotion has it long-last of contribution; But organize because of being subjected to aging, the personnel, accounting system comes to a deadlock etc. the tie of the factor, and can't meet an emergency the health insurance total amount system under of the medical treatment competes the condition of the pressure and the official business budget short. In many transformation projects with the mode of the government corporations compares for feat, hope the primary health stations primary health stations after hire social expert's scholar to establish a board of directors; The novice doesn't have public official's status; Inspect an organization to change from the before the event direct to after the event to investigate on the accounting, purchase and it promotes a national health policy to promote with the concert retention of the medical treatment policy in the public hygiene; Having the help of the substance to the increase of the primary health stations competition ability also can ease government of finance burden.
