摘要: | 面對死亡總有很多的想像與猜測,無論是上西方的天堂或東方的極樂世界,似乎都是生者在面對親人死亡時的盼望,期待親人是去一個美好的仙境,不會是下地獄上刀山、下油鍋,這也隱藏著自我安慰的想法。死亡時時刻刻都存在,尤其是在身處重症病房的護理人員,常常與被迫與死亡相遇,在現在這醫療環境體制下,沒有人可以和他們談論死亡,重症護理人員在面對自己親人死亡時的調適歷程也被忽略。本研究在於探討重症護理人員面對親人死亡悲傷調適歷程。 本研究訪談三位重症護理人員,以敘事研究方法呈現生命故事並進行跨個案分析與討論,研究結論如下:壹、重症護理人員面對親人生病重症的因應歷程,包含,一、護理人員理所當然的成為親人的醫療主要決策者;二、因家人重症住院而神經緊繃;三、尋求信仰的支持;四、在親人生病後,更能理解病人的需求。貳、重症護理人員面對親人死亡的悲傷歷程及調適,包含,一、面對病人死亡的反應:(一)病人死亡想哭但不能哭、(二)初遇病人死亡,對於病人的死感到歉疚、(三)家屬對病人死亡的反應會引發悲傷、(四)病人死亡的模樣,會引發對於死亡的想像與焦慮。;二、面對親人死亡的悲傷調適歷程,包括(一)選擇讓家人善終,不予施行心肺復甦術、(二)與悲傷面對面、(三)回到親人過世的醫院上班,時常觸景傷情、(四)對於親人死後的探尋及掛念、(五)周遭同事及朋友的暖心支持、(六)將感謝病人的教導化為對家屬的支持力量。叁、重症護理人員親人死亡後,開始尋找生命意義的旅程,包含:開始尋找自己的生活目標,珍惜現在擁有的人事物,開始學習觀照自己。 When people face the death, there is always a lot of imagination and speculation. Whether it is a heaven in the west or a paradise in the east, it seems that all living people are looking forward to the death of their relatives. Everyone hopes that after the death of their relatives, they can all go to the heaven in the west or the paradise in the east. Everyone will expect their relatives to go to a wonderful fairyland, they will not go to hell, take the road full of danger or go to the oil pot. These all hide the idea of self-consolation. Death is omnipresent , especially ICU nursing staff often encounter forced death. In the current medical environment, no one can talk to them about death, and their adjustment process in the face of the death of their relatives is also ignored. In this study, we explore the Grief Adjustment Process of the ICU nursing staff when facing the death of their relatives. This study interview three ICU nursing staff and present their life stories throughnarrative research and conduct cross-case analysis and discussion. The conclusion of this study is as follows: 1. ICU nursing staff face the pressure and adjustment of their relatives ill , including: (1) The nurse becomes to be the main decision-maker of the medical care of relatives. (2) Family members are ill critical and hospitalized, they will be nervous. (3) They will seek religious support. (4) After their relatives are sick, they can better understand the needs of their patients. 2. ICU nursing staff are facing the pressure and response of patient death, including: (1) Their response to the death of the patient including: a.When patient dies, they want to cry but cannot cry. b.Facing the patient's death for the first time, they feel sorry for the patient. c.When the patient's families respond to the patient's death, they cause their own grief. d.The looks of the patient's death will trigger their imagination and anxiety about death. (2) ICU nursing staff face to the Grief Adjustment Process of the death of their relatives, including: a.Let the family die peacefully without CPR. b.They can face Grief. c. They returned to the hospital where their relatives died, and they often feel sad. d. They miss them and want to know where their relatives will go after they die? e. Colleagues and friends around them will warmly support them. f. They are grateful for the patient's guidance and transformed it into a supportive force for their families. 3. After the relatives of ICU nursing staff died, they began to look for the meaning of their lives. They began to look for their own life goals, cherish everything, and began to learn to take care of themselves. |
