    Abstract:   本論文旨在探討第一次入住照護機構罹病老人的初期生活調適經驗。研究者透過深度訪談四位研究參與者及田野參與觀察蒐集文本資料,再以詮釋現象學方法為導向,針對文本加以分析及詮釋,瞭解第一次入住照護機構罹病老人在罹患慢性病的生活世界中普遍的置身處境、身心感受,以及他們在遷至機構居住後所面臨的生活調適問題。   研究發現,一、在強大的生物醫學模式下,經常遮蔽慢性病病人不顯見的意義本質;二、罹病老人透過以身體為根基,在時間、空間及關係向度上感受到多重缺口的呈顯及磨難;三、罹病老人在遷至機構居住後,找不到「位置」;四、生病的形體與機構的觀感仍然難逃污名的枷鎖;五、照護機構罹病老人在邊界處境下會引發個人的死亡焦慮及存在困境;六、第一次入住照護機構的罹病老人視機構生活為一個複雜的情蘊世界。   本研究根據研究結果所提出之建議,希望能有助於長期照護領域在法令政策制定、臨床護理實務工作、學校教育等層面有實質的助益;再者,也冀望能提供未來的研究及相關工作者之參考。
      In this study the main topic of discussion is how ill elderly initially adjust their lives inside institutional care centers. The information from this study was gathered from interviews with four researchers and observing field work dealing with the elderly. Hermeneutic phenomenology was used as the research method and the study was analyzed to understand how the ill elderly with chronic diseases felt both mentally and physically while staying in the care centers as well as any problems they faced after changing residences.   The study showed that :1.Under the biomedicine pattern chronically ill patients are for the most part not significantly observed;2.Ill elderly encounter hardships with time spending and having spaces showing on their bodies;3.The elderly are unable to find their own “place” after moving into institutional care centers;4.Their physical state and impression of the care center leaves the feeling of being in confinement;5. There is an anxious feeling of death and a difficult existence under the restrictive nature of care centers;6.First time elderly moving into the care centers see it as being a complicated place to understand.   The proposal based on this research will result in helping long termed care and essential benefits within law stratification plans as well as on policy creation. Practicing clinical nurses and institutional education can furthermore provide future reference for research and correlation of workers on this subject.
