    摘要:   隨著醫藥科技的發達、人口結構的改變趨向老年化及國人飲食習慣速食化及精緻化的改變,糖尿病患者逐年增加且年齡有年輕化的趨勢,國人及老人的十大死因中糖尿病都榜上有名而且都在五名內,糖尿病是一長期需要飲食、運動及服藥控制的慢性疾病,需長期就醫及生活習慣的調整才得以控制病情,中國的孝道文化,父母生病由子女照顧是天經地義的事,而在長期照顧過程中家庭的支持是很重要的,但亦有發現由子女為主要照顧者的患者其血糖控制較不理想。  本研究主要探討罹病者及主要照顧者(子女)在疾病適應過程及照顧歷程中的感受、困境及溝通、解決方式。透過立意取樣選取門診就診個案及其主要照顧者,罹病者及主要照顧者分別進行半結構式深度訪談,以詮釋現象學研究法,了解現象本質展開經驗脈絡的探討。  研究結果發現病患對於罹患糖尿病會以「馬上接受」、「對親友隱瞞」以維持健康形象,生活習慣調整但無法配合飲食控制,足夠的決定權是必要。主要照顧者在家戶長掌權下的順從照顧,尊重父母親人生觀無奈的妥協,以關懷、陪伴代替責備、強迫。子女的照顧對疾病調適心理大於生理,讓患者以自己的意願過想要生活,而造成血糖控制不理想。期望本研究結果可成為未來醫療院所、病患及其主要照顧者之經驗參考。
      Due to the impact of aging population and changing habit of fast-food and processed-foodoriented diet, the number of the patients with diabetes in Taiwan keeps on increasing over the years. However, the age of the patients keeps turning younger each year. Diabetes has become one of the nation’s and elderly top ten causes of death; moreover, it is also in the top five of both rankings. Diabetes is a chronic disease that needs long term medical treatment, diet and exercise plan to control the situation of the disease. It also requires long term medical support and change in personal habits of the patients. In the Chinese culture of filial piety, it is children’s duty to take care of parents in illness. Thus, family support is critical to the long term process of taking care of parents. However, few researchers discover that parents with diabetes have poorer control of blood sugar when they were being taken care of by their children.   This study aims to discuss the adjustment process of the illness between patients and their children as main caregivers. It also aims to examine their feelings, the difficulties they faced, their communicating process, and finally, their solutions to problems when taking care of the patient. The participants, including the patients and the main caregivers, were selected through purposive sampling method. Sami-structured interviews of the patients and caregivers were also conducted. In order to understand the natural phenomenon and the experience context of the data, hermeneutic phenomenology was adapted as the main analytical approach.  The findings of the study indicate that patients responded to diabetes with ‘Immediately accepted’ and ‘Hide from family and friends’, in order to build the false image of healthiness. The main caretakers obeyed orders of taking care of their parents and compromised to the life choice of their parents reluctantly due to the power given to the head of the family. Care for and companionship to these caretakers should be encouraged in order to replace blame and coercion. Their main adjustment in facing the disease was mainly psychological rather than physical. Also, the main reason of poor glycemic control was the loose self-control of lifestyle of patients. This study hopes to benefits the future medical experience of medical institutions, patients and their primary caregivers.
